Guidelines for Certificates and Resolutions:
A two (2) week notice in writing is required for all requests. The following information should also be included:
Date of event or presentation
Name of Individual or Organization to whom the certificate is for
Due date
Contact name and phone number in case there are any questions regarding the request
Sample wording is appreciated for certificates (required for resolutions)
